REQUEST FOR PROPOSALS

 

 

Hampden Township is soliciting proposals from an established ambulance billing company to provide ambulance billing, revenue recovery, financial reporting, compliance, and additional services as needed.

 

The proposal must be submitted to:

no later than 4:00 p.m. on Monday, May 28, 2013.  One (1) original and three (3) copies of the original  proposal and one (1) complete electronic copy in Microsoft Word or PDF format burned to a disc or saved to an external drive (USB drive) are requested from each bidder.  The sucessful bidder will be required to post a performance bond in the amount of 100% of the accepted bid amount.

 

The necessary information packages may be picked up at the Township Building, Monday through Friday, between the hours of 8:00 a.m. and 4:30 p.m., or may be done by clicking here.

REQUEST FOR PROPOSALS

 

 

Request for Proposals for EMS Billing Services for Hampden Township Emergency Medical Services should be submitted to Hampden Township at the address below no later than 4:00 p.m. on Tuesday, May 28, 2013.  The bid opening is to be held at 12:00 p.m. on Wednesday, May 29, 2013 at the Hampden Township Municipal Building at 230 S. Sporting Hill Road, Mechanicsburg, PA 17013.

Invitation to Bidders

 

The Hampden Township Board of Commissioners, Cumberland County, 230 S. Sporting Hill Road, Mechanicsburg, PA 17050-3097, will accept sealed proposals until 2:45 p.m., May 29, 2013.  All bids will be publicly opened and read aloud on May 29, 2013, at 3:00 p.m., at the offices of Hampden Township noted above by the Director or Assistant Director of Public Works. 

 

2013 Street Pavement Reclamation and Resurfacing Project

Contract No. 2013-02

 

Proposals must be submitted upon forms furnished by the Township and must be sealed with proper identification of subject bid on the outside of the envelope.  All bids must be accompanied by a certified check or bid bond in the amount of ten percent (10%) of the bid.  Instructions, specifications, and bid forms may be obtained at the Hampden Township office at the above address from 8:00 a.m. to 4:00 p.m., Monday through Friday, except holidays or by clicking here.  The successful bidder will be required to post a performance bond in the amount of 100 percent (100%) of the bid price.

 

The Board of Commissioners reserves the right to reject any or all bids or parts thereof and to waive any defects or irregularities in the best interest of the Township.  Unless all bids are rejected, the lowest responsible bidder may be selected at a public meeting of the Board of Commissioners to be held May 30, 2013 at 7:30 p.m. in the Hampden Township office.
